How to Change Your Primary Care Physician (PCP)
1. Find the member services telephone number on your insurance card, usually it is on the back and is usually a toll free number.
2. Call the member services telephone number and tell them you want to change your PCP.
3. Be prepared to provide the following information to your insurance provider:
- Insurance ID – usually located on the front of the card
- Name of PCP – Dr. Kristen Prescott
- Address – Prescott Pediatric Care, P.C., 171 Daniel Webster Highway, Suite 2, Belmont, NH 03220
How To Prepare for Your Appointment
1. Prior to your appointment, make sure you have changed your PCP
2. If this is your first appointment - get, or request your medical records for all patients
3. Have your insurance card for all patients with you
4. Know and be prepared to provide your co-pay upon arrival – usually the co-pay amount is on the front of your insurance card
How To Obtain Your Medical Records
1. Try and give yourself at least 30 calender days in advance of your need to have your records
2. Complete this form and fax or deliver it to the practice you are requesting your records
3. Please ensure you request your entire medical record, this includes, but may not be limited to:
- Previous copies of medical records from previous provider organizations that you received when you moved your PCP to your last providing organization
- Shot records – these are especially important and you should ensure they are with your medical record
- Laboratory and other results provided by specialists
Before you call the New Hampshire Medical Board at 603.271.4161, please make sure you have tried to resolve the matter, take the name and phone number of the person or persons you have spoken to, the date or dates you spoke to them and the reason you are told you cannot have your medical records.
