How to Change Your Primary Care Physician (PCP)

1. Find the member services telephone number on your insurance card, usually it is on the back and is usually a toll free number.
2. Call the member services telephone number and tell them you want to change your PCP.
3. Be prepared to provide the following information to your insurance provider:

  • Insurance ID – usually located on the front of the card
  • Name of PCP – Dr. Kristen Prescott
  • Address – Prescott Pediatric Care, P.C., 171 Daniel Webster Highway, Suite 2, Belmont, NH 03220

How To Prepare for Your Appointment

1. Prior to your appointment, make sure you have changed your PCP
2. If this is your first appointment - get, or request your medical records for all patients
3. Have your insurance card for all patients with you
4. Know and be prepared to provide your co-pay upon arrival – usually the co-pay amount is on the front of your insurance card
